2025-2026 Children's Faith Formation Registration

Children's Faith Formation classes K-6 will be held on Sundays from 8:45 a.m. to 9:25 a.m. Classes for grades 7/8 and 9/10 will be held on alternating Sundays from 4:00 p.m. - 5:45 p.m. Please complete this form as part of registering your child for our Faith Formation Program. Faith Formation Program Registration Fee

The registration fee is $200. The payment link will be included in the confirmation email you receive after completing this form. Please note that your child's faith formation registration is incomplete until payment is received.
